Hey Neno, if the man is running through the town at night or early morning, then there should not be many people about regarding reactions in the film of passers by when you film it, unles you are wanting this then your extras should be briefed how to react,. when they see a man with an axe running past. if your man is thinking about doing something to his ex eg "Kopf kino", then you can film such scenes where he goes to her house and kills her etc or not. but at the end you dont know did he did it or was he just thinking about it. but cutting to these scenes during the video. For placement scenes and wide angle then a drone would be good but filming from high locations in building or from bridges etc could be more controllable and no need for permission to fly etc.
you have 4 subjects, 1 the man, 2 running, 3 axe, 4 town. so what is your main focus, the man, he is the main subject, who is he what is he doing and why, the running, he needs to get somewhere fast for some reason, urgency, anger, pain, is causing this? the town, this is your backdrop you see that man running through this, so you can cut to cool scenes of things he is running past, maybe in the film at first he is carrying a package and you do not see what is inside or a bag, then halfway through the film as he is running he opens the package or it accidentally rips open and you then see the axe, then the first feeling of the running man has changed. then you can fixate on the axe, maybe he is running with it dragging along the ground causing sparks, he has it over his shoulder, or is carrying in both hands or trying to conceal it while he runs if at the end there is blood on the axe when he is cutting the wood would be a clear indicator that something sinister.
a couple of videos for inspiration "the verve bitter sweet symphony" it is just a man walking to camera but the impact on his behavior on the passers by holds the focus rather than the man walking. you dont see much of the town and he is not running and he doesnt have an axe but it is powerful. all extras are briefed on how to act when hit.

this one is just the runner but the different locations and angles and look at the cut speed. to the music. focus on the man and the location but no axe.
i found this from Michael Penn for his song Try, all filmed in one shot which is quite an interesting concept but not always easy it has to be rehearsed and if you get
anything wrong you have to start from the beginning.
this is another running video the different speeds, to focus on the runners physique, or what they are wearing or where they are running.
then later the 3 runners in the city. may give you ideas on how to film it.
